Why Women's Health Research Remains Underfunded

NIH funding for women's health research lags disease burden by staggering margins.

Congress asked for a number, and in February 2025 the National Academies of Sciences, Engineering, and Medicine handed them one that's hard to sit with: 8.8%. That's the share of NIH research spending between fiscal years 2013 and 2023 that went toward women's health, and the report notes that share actually shrank as a proportion of NIH's total even as the total kept climbing. Eleven years of data, and the line still goes the wrong way.

Put dollars on it, because a percentage this small is easy to wave off as a rounding error. NIH spent something like $4.6 billion on women's health in FY 2024, about 10% of its total appropriation, and that figure edges toward $5 billion in FY 2025. Sounds like a lot of money until you set it against what the NASEM committee actually recommended: a new, dedicated NIH institute, plus $15.7 billion in fresh congressional funding over five years. Split that $15.7 billion and you get roughly $4 billion to build the institute, $11.4 billion into an NIH-wide fund for interdisciplinary work that doesn't fit neatly into any single existing box.

It took me longer than I'd like to admit to understand what the committee was actually flagging. The claim isn't simply that women get less funding overall. Diseases hitting women harder get funded at lower rates relative to their burden than diseases hitting men harder, which is a more specific and more damning claim. The 8.8% is a thermometer reading, nothing more, and it only tells you the patient's running a fever. Writing the prescription is a separate job, and Congress hasn't shown much hurry to pick up the pen.

What disease-by-disease comparisons reveal about where the gap cuts deepest

Endometriosis is where this stops being abstract and starts being infuriating. It affects an estimated 190 million women worldwide, takes eight to nine years on average to diagnose, and pulled $28 million from NIH in FY 2024. HIV/AIDS research got $3.1 billion that same year. Endometriosis affects more than five times as many Americans as HIV does, and it still lost that comparison by two orders of magnitude, which is the kind of gap you have to read twice to trust. Stack it against prostate cancer instead: $311 million in 2024, more than eleven times the endometriosis figure, and nothing in prevalence or economic toll explains an eleven-to-one split. Run the 2022 numbers on a per-patient basis and endometriosis research comes out to about $2 per patient per year.

PCOS tells a stranger story, because underfunding isn't even the worst part of it. Researchers are leaving the field entirely. NIH grant applications for PCOS research dropped 42% between the 2006-2010 window and 2011-2015, from 74 down to 43, while applications for comparable conditions held steady or grew. Researchers here are quietly deciding not to apply at all, a different and more corrosive kind of loss than simply losing a funding decision.

Chronic fatigue syndrome, ME/CFS, skews heavily female and carries a burden-to-funding ratio of 0.04. HIV/AIDS sits at 15.6 on that same scale, and those numbers being in the same universe as each other is worth pausing on. Autoimmune disease runs a similar pattern: women make up close to 80% of everyone diagnosed with an autoimmune condition, and rheumatoid arthritis research in women still only pulls about $6 million a year. One analysis put the return on doubling that funding at something like 174,000%, a number that holds up on a second and third read. The money still hasn't moved, which tells you the holdup was never really about the math in the first place.

Look at the whole list together and a shape emerges: high prevalence, high burden, thin funding, and the gap widens the farther a disease sits from the reproductive organs. Endometriosis at least gets called a "women's disease" out loud, even if grudgingly, while ME/CFS barely gets called a disease at all.

How a 1977 FDA guideline shaped decades of medical knowledge

None of this happened by accident, and most of it traces to a single regulatory decision from 1977. In response to concerns about fetal safety, regulators moved to limit women of childbearing potential from early-phase clinical trials. The intent was protective. Nobody sat down and planned the next two decades of exclusion that followed from it, and almost nobody stepped in to correct course once it was underway.

Technically the guideline only covered phase 1 trials, the earliest studies where researchers figure out dosing. Drug sponsors, reading a rule that gave them plenty of room to be cautious, ran with it well past its letter and kept women out of later-phase trials too, the ones that actually determine how a drug behaves in the population that will eventually take it. Women stayed mostly absent from clinical research until the 1990s. Meaningful federal requirements for inclusion and sex-disaggregated data took years longer to arrive, and that gap is worth sitting with. Twenty-one years passed between the exclusion and the first real attempt at correction, and that gap is worth sitting with.

Fixing the rule didn't fix the practice, and that distinction matters more than it should. A 2022 study covering 1,433 trials and more than 300,000 participants found women made up only 41.2% of participants on average, decades after the rules supposedly changed. In industry-sponsored early-phase trials today, fewer than 30% of participants are women. Even where women do get enrolled, plenty of studies still don't break results down by sex to check for differences between the two groups. Inclusion without analysis leaves the actual gap standing exactly where it was; the bodies are in the room while the question about them just never gets asked.

Once you trace the compounding math, the pattern gets almost embarrassingly circular: less data on women meant less research interest, less interest meant less funding, less funding meant less data. Nobody designed that loop on purpose, and it just kept feeding itself because nobody outside the system had a reason to interrupt it.

The medical consequences of building drug knowledge on male bodies

So what happens after fifty years of pharmacology gets calibrated to a body that represents half the population but does a fraction of the enrolling? Women experience adverse drug reactions at roughly double the rate men do, and a real chunk of that gap traces straight back to trials run mostly on male participants. One pharmacokinetic analysis of 86 drugs found that 76 of them produced higher blood concentrations and longer elimination times in women. The drug just sits in the system longer, at higher levels, than the dosing was ever designed to expect. That's the difference between a therapeutic dose and something closer to an overdose, and it sat there unmeasured for years because nobody thought to check.

Between 1997 and 2000, the FDA pulled 10 drugs off the market, and eight of the ten came down because of risks specific to women. In documented cases, sex-based dosing adjustments only emerged years after drugs were already in widespread use, once the real-world consequences for women became impossible to ignore.

Misdiagnosis compounds the drug-safety problem instead of sitting off to the side of it. Misdiagnosis rates for serious cardiovascular events are well documented to fall harder on women, a pattern that persists despite cardiovascular disease being a leading cause of death across sexes. Pain treatment runs the same lean: research has found women are systematically less likely to have their pain assessed promptly, wait longer to see a doctor, and receive pain medication.

None of that is coincidence, and none of it is mysterious once you know the history. It's the direct, traceable output of a research base that never treated female physiology as a variable worth measuring in the first place.

Why the funding gap keeps reproducing itself structurally

Why does this survive year after year, report after report, without ever really closing? A handful of mechanisms feed each other, and pulling on just one barely moves the rest of them. That's worth walking through slowly, because the individual pieces look almost reasonable in isolation.

Start with definitions, since a lot of the damage happens right there, before a single grant even gets written. "Women's health," as both a funding category and a phrase people actually use, gets treated as nearly synonymous with reproductive and maternal care. That framing leaves out autoimmune disease, cardiovascular disease, ME/CFS, and anything else that disproportionately affects women without involving pregnancy or ovaries. Draw the category too narrow, and the funding follows that narrow line rather than the disease burden sitting just outside it.

Stigma does its own quiet work underneath all of this. Conditions that mostly affect women, menopause, endometriosis, chronic pain syndromes, have carried a cultural weight for decades that makes researchers and institutions less eager to study them in the first place. That shows up downstream as thinner grant pipelines and less institutional appetite for the work, long before anyone's even drafted a proposal.

Then there's the matter of who's doing the asking. Female researchers have consistently received a minority share of NIH research grants over the study period. Fewer women running labs means fewer people positioned in the room to design, and fight for, studies centered on female physiology. Call it a pipeline problem stacked on top of a funding problem: each one quietly props the other up, and neither one moves first.

Data fragmentation closes the loop. Without sex-disaggregated numbers, the real burden of a female-dominant condition gets undercounted, which makes the funding case that much harder to build in front of an institution or an investor committee. The economic argument alone should carry more weight than it does. The economic costs of untreated women's health conditions run far ahead of the federal funding directed at them, a gap that should make the investment case straightforward. That gap looks absurd until you remember the cost is diffuse, spread thin across millions of individual women, while the institutions making funding decisions never see the bill land on their own desk. Women make up roughly half the U.S. population, tend to live longer than men on average, and spend a disproportionate share of those additional years in poor health.

Historical exclusion thins the research base, and a thin research base feeds the stigma. Stigma keeps women out of the grant pipeline, and a male-heavy pipeline keeps definitions narrow, which keeps burden undercounted. Small fixes at the margins were never built to touch a structure wound that tight.

How the 2025 policy environment is widening the gap further

The last section describes a slow-moving problem, decades in the making. 2025 dropped a fast-moving one right on top of it. Reports covering activity through mid-2025 documented substantial cuts to grants specific to women's health and economic security, running into the billions. That's a cut into a field the NASEM report had, just months earlier, already called deeply inadequate.

The timing alone is worth sitting with, because it's almost too on the nose. NASEM calls for $15.7 billion in new money over five years; within the same calendar year, the policy environment moves in the exact opposite direction. What happens to the people who would have relied on that research once it's simply gone?

The vacuum doesn't stay empty; it gets filled by something else. When funded research disappears from credible institutions, women don't stop looking for answers about their own bodies; they turn instead to social media and informal networks, sources that vary wildly in reliability and were never built to carry the weight of clinical evidence. That's the predictable result of pulling institutions out from under a problem and hoping the problem solves itself in the meantime. It won't, and it hasn't, because the structural forces from the last section need active counterweight just to hold steady, and 2025 policy pulled even the modest counterweight that existed off the table.

What closing the gap would actually require

The NASEM report is, at minimum, the most specific public blueprint anyone's put on paper: a dedicated NIH institute, $15.7 billion in new appropriations over five years, a mandate to fund conditions by actual disease burden instead of fifty years of habit. Call that the floor. Treating it as the ceiling would be its own kind of mistake, and arguably the same kind of complacency that got us here in the first place.

Money alone won't move much without fixing the definition problem first. "Women's health" has to mean more than reproductive and maternal care if the field is ever going to cover the conditions where sex and gender actually shape biology and outcomes, which turns out to be most of medicine once you look closely enough at it.

Clinical trials need their own fix, and it's less about who gets enrolled than what happens to the data afterward. Enrollment without analysis, the problem from a few sections back, leaves the knowledge gap standing even with the right people sitting in the waiting room. Mandatory sex-disaggregated analysis, paired with inclusion, is what actually builds an evidence base solid enough to justify the next round of investment.

Private capital has its own repair job overdue, too. Endometriosis, PCOS, and menopause represent enormous patient populations with real, documented unmet need, and the fact that they capture under 2% of private healthcare funding looks less like a moral failing at this point and more like a market badly mispricing risk and demand. Somebody's leaving money on the table, and that somebody is attached to a few hundred million patients who would very much like better answers, preferably sooner than another fifty years from now.

The researcher pipeline matters as much as any single dollar figure above, even though it gets less airtime. Closing the R01 gap and getting more women into senior research roles changes which questions get asked in the first place, and which of those questions survive long enough to actually get funded.

One new awareness campaign or one fresh grant program was never going to close a gap like this. The mechanisms are woven too tightly for a single thread to unravel the rest of the knot: it takes funding, regulatory requirements, and definitions moving together, not in sequence, not one at a time while everyone waits politely for the others to catch up. Pick one lever and pull it alone, and the honest prediction is that somebody writes this same report again in fifty years, probably with a different percentage attached and the same shrug waiting at the end of it.
